How to Register for Baptism

We’re honored that you’re considering baptism at AME Fountain Chapel. Baptism is a sacred step of obedience, dedication, and new life in Christ.

To begin the process:

  1. Download the official registration form using the link below.

  2. Print and fill it out (one form per person being baptized).

  3. Return the completed form by either:

    • Emailing it to info@amefountainchapel.org

    • Dropping it off at the church office:
